Useful information

HOME VISITS

Patients can be visited at home only if too ill to attend surgery.  More detailed examination and tests can be performed in the surgery rather than at home.  All home visit requests are triaged by the doctor and acted upon accordingly.

REPEAT PRESCRIPTIONS

Please order repeat prescriptions 7 days before expiry of supply to allow time for processing Patients on regular medication may obtain repeat prescriptions. Please deposit your request for a repeat prescription in the box provided at reception.  Please allow 2 FULL WORKING DAYS BEFORE COLLECTION.  Prescriptions can be picked up at reception during office hours.  If required, prescriptions can be posted back to you on request by enclosing a stamped addressed envelope.  If you are dropping repeat prescription request through the letter box, please use an envelope.

REGISTRATION PROCEDURE

New patients should bring with them their signed medical card or complete a registration form at reception.  Registrations will not be complete unless you complete the questionnaire provided and attend a brief health check with the treatment room nurse at Great Harwood or practice nurse at Rishton, followed by an introduction appointment with doctor.  We accept patients living in Great Harwood and Rishton. The Doctors would like to see you again 3 weeks after registration once your previous records have been received.

 SUGGESTIONS AND COMPLAINTS

The practice welcomes comments and suggestions from patients.  If you have a complaint please talk to the practice manager.  Alternatively you may address a letter of complaint to the practice manager or to doctor. You will be given a copy of the complaints procedure within the practice detailing the procedure and a form to express the complaint.
If you are not satisfied with the way your complaint has been handled please contact NHS East Lancashire Customer Care Department 01282 878653.

 

ZERO TOLERANCE

The practice will not tolerate any verbal abuse or violence towards any member of staff.  Abusive and violent patients will be removed from the practice list after due warning.

RIGHTS AND RESPONSIBILITIES OF PATIENTS

All patients will be treated equally regardless of race, gender, sexual preference or disability.  All patients are expected to abide by practice rules by being on time and keeping appointments and attending clinics when requested by the doctor.
If the practice does not provide a service, we will offer an alternative provision.
If you cannot keep an appointment for any reason please ring the surgery at the earliest opportunity to cancel so the appointment can be offered to another patient.

PRACTICE USE OF PERSONAL HEALTH INFORMATION

Please be assured that all patient information provided to the doctor, nursing, or reception staff is treated with the strictest confidence.
When you see the doctor, the information you provide during the consultation is logged onto a secure computerised notes system which enables the doctor to keep track of progress with your problem for example with referrals to hospital specialists.If you attend a hospital clinic, letters from the specialist clinic will be kept as part of your paper records which is handled with the strictest confidence.
